Agency Referral Form 

This form is for use by staff at organisations who wish to refer potentially ‘at risk’ clients to the Fire Services in Wales for a Home Fire Safety Check.

In the meantime you can still refer a client by calling us or by faxing one of our referral forms. If faxing the form please indicate who the Fire Service should contact to arrange the check e.g. case worker, family member, as sometimes it may not be appropriate to contact the client directly.

Referrals can also be made directly to the regional Fire Services.
