Client Referral Form

This form can be used for recording the details of a client you wish to refer for a Home Fire Safety Check. The form allows us to capture additional information on particular circumstances or conditions of the client which will assist the Fire Services in preparing for the check.

A referral can be made using the online referral form, by faxing this form to Firebrake or by telephone. Referrals can also be made directly to the regional Fire Services.

If faxing the form please indicate who the Fire Service should contact to arrange the check e.g. case worker, family member, as sometimes it may not be appropriate to contact the client directly
